Health plan coordination
MNPrairie provides Health Care Coordination through two Managed Care Organizations, South Country Health Alliance and Blue Plus.
South Country is a county-based purchasing health plan that partners with counties to support accessible, quality health care through partnerships with community services and local health care providers for Minnesota Health Care Program enrollees. South Country partners with MNPrairie in Dodge, Steele, and Waseca counties to provide Care Coordination or Care Connector services to assist members with locating and obtaining the services and support they need.

Blue Plus is a Managed Care Organization, available throughout the state of Minnesota, that provides health plan options for individuals that qualify for Minnesota Health Care Programs. Blue Plus partners with MNPrairie in Steele County to provide Care Coordination services to assist Elderly Waiver members with locating and obtaining the services and support they need.
