Minimum requirement for an application

Your food support benefit start date is based on the day that you submit your application.  To set that start date, you must submit Page one of the Combined Application Form (CAF), including your signature and the date. The day the application is received by the agency is the first day you may be eligible to receive benefits. You have 30 days to submit the rest of the Combined Application Form.

You will participate in an intake interview, either in-person or by phone for our agency to determine eligibility for the SNAP program.

To set the date submit one page of the CAF including the following:
  • Name
  • Address
  • Sign and date
You have 30 days to submit the rest of the CAF
