Health plan care coordination
South Country Health Alliance (SCHA) is a county based health plan. It is considered a managed care organization. Through our delegation agreement with South Country Health Alliance, MNPrairie provides care coordination services to SCHA members enrolled in the following programs: Minnesota Senior Care Plus, Senior Care Complete (MSHO), Single Care (SNBC), Ability Care.

Mental health
Adult mental health (AMH) services for MNPrairie are provided through contracts with private agencies, where a wide variety of services can be obtained. We can provide connections to services and resources, in the county and in the community that will put you on a path to live a full and meaningful life.

MnCHOICES is a person-centered assessment to help people with long-term or chronic-care needs make care decisions and select support and service options.

Senior Services
Our services help seniors who reside in Steele County live as independently as possible. We work to make sure seniors and their families know the resources available to them from the county, state, and federal government. If you live in Dodge or Waseca Counties please contact your Public Health department to seek assistance.

Disability Services
Our services help people with disabilities live in their communities as independently as possible. MNPrairie can help with short-term service planning and ongoing case management. We can help people with disabilities and their families to get access to tools to help them live independently or get the help they need.

Substance use
Substance Use (SU) services are available to MNPrairie county residents who have problems with drug and/or alcohol abuse and meet the income guidelines, or have been referred by the court for services. Assessment services can be found within MNPrairie offices and also from a variety of healthcare and substance use provider agencies.

Adoption
MNPrairie provides adoption case management and services to children who are under our jurisdiction. By this, we mean children who have become available for adoption because of a child protection process and are already in foster care.

Housing navigation & eviction prevention
• Eviction Prevention services, including support of households to resolve the housing crisis through activities such as outreach and engagement, assessment, connection, coordination, housing counseling, conflict mediation, personal and legal advocacy, and follow-up. • Housing Navigation services that help households find or obtain stable housing, including assistance identifying potential housing opportunities, application assistance, and landlord outreach and engagement.

Housing Stabilization Services is a new Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing.

Information for providers
Housing Support is a state-funded program that provides an income supplement each month to pay for rent, utilities and household furnishings for eligible individuals and families. The MNPrairie service area has access to state-funded Housing Support which include a supplemental service rate. A supplemental service rate may cover the following: transportation; help to navigate services for their physical health, mental health, or chemical health; and other case management type supports.

Burial assistance
When families need help arranging their loved ones' final resting place, MNPrairie can help. The Burial Assistance Program can assist with final expenses when families don't have the resources to do so themselves. MNPrairie workers will help you assess your eligibility for burial and other related expenses.

Cash assistance
MNPrairie offers a number of programs to supplement families' and individuals' incomes. The type of assistance people receive depends on their life circumstances, family size and type, and other factors. We can help you find the program that works best for you.

Child care assistance (CCAP)
The child care assistance program helps families pay for child care so parents can work, look for a job or attend school. The program is available for children younger than 13 and for children with special needs who are younger than 15.

Emergency assistance
When low-income households are in a financial crisis and are at risk of losing their housing or are up for disconnection or already disconnected from utilities service providers, the MNPrairie Emergency Assistance Program (EAP) may be able to help by providing short-term financial assistance.

Food assistance
The Supplemental Nutrition Assistance Program (SNAP) helps people with low income to buy the food they need for sound nutrition and well-balanced meals. The program issues benefits electronically on a card that is used to buy food at local stores.
